Incredibly helpful book for the new nurse or the experienced nurse who wants to improve her charting. This book is very easy to understand and provides helpful charting examples.

I found this book useful and and good review for documenting.

I loved this book. Unfortunately, my cat didn't and knocked a drink all over it. I will probably purchase it again if I go to another facility that needs narrative charting. Great examples of what is include in your documentation, and had a wide range of topics to choose from with an index.

the book is a good overview of the different styles of charting used, but lacks detail into any particular method. I hoped it would have been more useful.

This is a very easy book to read and use the sample which are shown.This book was use in a class room of new nurses learning how to chart and we know how hard that can be.

This book pretty much will review all of the do's and dont's of charting that you learned in nursing school. I am a new nurse who wants help with my documentation wording. I don't need to know all of the concepts. I know enough to stay out of court. Its basically a common sense approach to documentation.

You really need the book not the book. It's hard to use
